    Documents required to Enroll

    Please upload the following documents or you may email them to the Data Manager after you complete the registration process.

    • Legible certified copy of child’s birth certificate
    • Copy of valid photo ID or driver’s license of parent or legal guardian(s)
      • If guardian is not listed on the birth certificate, guardianship must be established by
        • Legal custody documentation papers OR
        • Affidavit of Caregiver (available from the Data Manager)
    • Proof of Residence (Must show guardians(s) name and physical address)
      • Mortgage Document or Property Deed
      • Residential Lease
      • Utility Billl (Must show guardian's name and service address)
        • Electric
        • Water
        • Gas
        • Cable
    • Health or Immunization records
    • NC Health Assessment (First-time enrollee – forms are located on the district website under Health forms)
    • Current Transcript or Report Card (new 9th Grader)
    • Homeschool documentation (if the child was homeschooled prior to entering public schools)
